Front wheel spacers

I coudnt think of a better title but I was curious if anyone has altered how the front wheels sit in so recessed on the first gennys. I do not know if spacers would necessarily be the answer but to me they just look so far in maybe because I have a dually just wondering if anyone has a remedy for this that is safe and reliable or if its not even worth messing with.
1993 dodge d350 2wd/dual . Converted auto to NV4500. DIY air intake, fixed all the leaks and now ready to upgrade
