FWIW, it looks like it ate something other than itself judging by that retaining nut. Did the air filter come apart or something?

It literally looks like someone decided to throw a handful of sand into it.

I was thinking either something got in there when working on it or possibly some big chunks of the compressor wheel bouncing around after the shaft bent.

How much of it made it to the engine? Or at least the intercooler.
RIP 91.5 W250 5 speed. The great experiment.
92 W250 HD 47RH with Compushift. Upgraded H1C to 62/60/16, Scheid Lightning VE, 60# valve springs, DAP 7X.010 injectors, 4" DE exhaust, home made cooler tubes.

Good point, when the turbos on our Macks explode we have to pull the intercooler and flush it out backwards, if you don't pieces will eventually work their way through and work their magic on the new turbine wheels.

I think I was pretty lucky, there were lots of small pieces in the intercooler and the piping on the turbo side. The intake side piping looked pretty much like normal. Imagine it sucked in lots dust sized particles though.

Just curious how many miles on your truck when the turbo failed?
1993 W250 Regular cab, 5speed Getrag (rebuilt), 4:10s, Warn hubs, 4" exhaust, 3200 rpm, big fuel tank in bed, Warn M10,000 winch,KDP fixed (Air conditioner recently fixed!!! )

I wouldn't blame that failure on the turbo, something got in there and did that.
